Practicioner Registration 

Tayfun Yuksel, Psychologist (also known as Tay Yuksel in this practice and on social media, such as the facebook and instagram 'Tay Psychology' pages), is fully registered with the Psychology Board of Australia. The full registration number is provided in all reports, letters, and invoices. Registration details can also be verified on the AHPRA website. 

 

Part 1: Terms and Conditions

 

1. Services Provided

Tay Psychology offers professional psychological services to adult clients across Australia via telehealth. Appointments and communication are managed through Halaxy, with all forms and client contact handled electronically.

 

2. Bookings and Reschedule/Cancellations

 

  • All appointments are scheduled via Halaxy.
     

  • A minimum of 48 hours’ notice is required for cancellations or rescheduling for therapy services.
     

  • Cancellations with less than 48 hours' notice may incur a fee equivalent to 100% of the session fee for therapy services.

 

  • Refunds are not available for assessment services. Please ensure this is a suitable service for you before proceeding with a booking.
     

 

3. Payment and Invoicing

 

  • Fees vary depending on the type of service and will be communicated before your first session.
     

  • A valid credit or debit card is required for all bookings. Payments are processed prior to your appointment automatically via Halaxy’s secure payment system.
     

  • Outstanding invoices or failed payments may result in additional fees and the suspension of future services until the balance is cleared.

 

 

4. Consent for Telehealth Services

By attending a telehealth session with Tay Psychology, you acknowledge and agree that:

 

 

  • You understand the potential risks of telehealth, including technical disruptions and unauthorised access, despite our efforts to maintain secure systems.

  • You will attend sessions from a private, distraction-free, and safe location.
     

 

5. Confidentiality and Privacy

All information shared during sessions is confidential and protected under Australian law. Your personal information will not be disclosed to others without your consent, except in the following circumstances:

 

  • There is a risk of harm to you or others.
     

  • Disclosure is required by law (e.g., subpoena, court order, or legal obligation).

 

 

Third-Party Disclosure

Where you have provided consent, we may share relevant information with third parties involved in your care. For example, if your GP, referring healthcare provider, or insurance case manager requires updates on your treatment, we may discuss progress, treatment plans, and other relevant details as appropriate.

 

Third-Party Information Sharing and External Links 

Where reasonable and practicable to do so, we will collect your personal information only from you. Our website contains links to external websites. If you choose to leave our site, we cannot be responsible for the protection and privacy of any information which you provide while visiting such sites, and such sites are not governed by this privacy statement. You should exercise caution and review the privacy policies of any external websites you visit.
 

6. Limitation of Liability

Tay Psychology is not liable for:

 

  • Service interruptions due to internet issues or third-party platform failures.
     

  • Any loss or damage resulting from technical disruptions or unauthorised access during telehealth sessions, despite reasonable efforts to ensure security.

7.  Telehealth Suitability:
Tay Psychology may determine that Telehealth is not appropriate for a client. If this occurs, the client may be referred to another provider for in-person (face-to-face or community-based) support.
 

8. Changes to These Terms

We may update these Terms at any time. The most current version will always be available on our website. Continued use of our services indicates your acceptance of any changes.

 

Part 2: Privacy Policy and Client Information

Tay Psychology is committed to protecting your privacy in accordance with the Privacy Act 1988 (Cth) and the Australian Privacy Principles (APPs). This section explains how we collect, store, use, and protect your personal information, and outlines your rights as a client.

 

1. Collection of Personal Information

We collect personal and health information necessary to provide psychological care, including:

  • Medical and psychological history, assessments, and treatments
     

  • Billing and payment information, managed via Halaxy
     

 

2. How Information Is Collected

  • Intake and consent forms
     

  • Online booking and communication platforms (such as Halaxy, Microsoft Teams, Zoom)
     

  • Phone, email, and other secure digital tools

    Some third-party platforms we use may store data on servers located outside of Australia. Where this occurs, we take reasonable steps to ensure that these providers comply with Australian Privacy Principles or equivalent data protection standards.

 

3. Use of Personal Information

Your information is used to:

 

  • Deliver psychological services
     

  • Manage appointments and billing
     

  • Communicate with you regarding care

4. Data Security

We use secure systems to manage and store your information. Halaxy complies with Australian data protection regulations, and all reasonable steps are taken to prevent misuse, interference, or loss of your data.

We retain client records in accordance with applicable Australian laws and professional guidelines. In most cases, records are stored for a minimum of 7 years from the last date of service (or until age 25 for clients under 18), after which they are securely destroyed or de-identified.

As a client of Tay Psychology, and under the Privacy Act 1988, you have the right to:

 

  • Access your personal information held by us and request updates or corrections if any of that information is inaccurate, out of date, or incomplete, subject to certain exceptions. If you wish to access your personal information, please contact us in writing. Tay Psychology will not charge any fee for your access request but may charge an administrative fee for providing a copy of your personal information. To protect your information, we may require identification before releasing it.
     

  • Withdraw from services provided by Tay Psychology at any time, with appropriate notice.
     

It is important to us that your personal information is accurate, complete, and up-to-date. We will take reasonable steps to ensure the information we hold is current. If you find that the information we have is inaccurate or outdated, please inform us as soon as possible so we can update our records.
If you have concerns about how your personal information has been handled, you may lodge a complaint with us directly. If you are not satisfied with our response, you may contact the Office of the Australian Information Commissioner (OAIC) via www.oaic.gov.au
